In Muay Thai, Carano started out as a professional athlete. Her ex-boyfriend Kevin Ross, a professional muay thai fighter, encouraged her to start training in the sport. Carano accepted an offer from Jamie Levine to join World Extreme Fighting in the first-ever sanctioned female mixed martial arts fight in Nevada to face Leiticia Pestova after scoring a record of 12-1-1 in Muay Thai. Carano’s opponent for the match was Leiticia Pestova (a fight Carano won in 38 seconds). She was given the opportunity to face off against Rosi Sexton at the World Pro Fighting exhibition in Las Vegas. Carano was victorious in the fight after defeating Sexton in the latter stages of the second round.
At the Strikeforce event titled “Triple Threat” on December 8, 2006, Carano faced off against Elaina Maxwell. She won the fight unanimously. Carano disproved many who doubted her by winning a decisive victory over Maxwell for the second time, her first triumph in a Muay Thai competition. After a months-long standoff regarding their free-agent status, Carano and other fighters from the now-defunct ProElite promotion were contracted to Strikeforce after Strikeforce bought ProElite’s assets. Carano was joined by other fighters in the ProElite promotion.
